Our Outreach Department at the Senior Center can assist you with: 

Information and Referrals for Seniors   -  Referrals can only be made with your permission, and Outreach respects the confidentiality of our work.

  • Referrals to BayPath Elder Services, Meals-on-Wheels, Visiting Nurse Associations, Eldercare lawyers, SMOC (South Middlesex Opportunity Council) services, food pantry, Advocates and other professionals and agencies as needed. 
Public Benefits Assistance
  • Applications with SMOC Fuel AssistanceSNAP (Supplemental Nutritional Assistance program,) tax exemptions, Mass. Health services, Supplemental Security Income (SSI), Social Security Disabilities Income, (SSDI)
Ongoing Case Management
  • Supportive visits as needed to help you maintain your independence, safety and health within your home and community.
        Note: BayPath Elder Care Service referrals are done for light homemaking and/or personal care.  Money management can assist you with monthly bill paying with trained volunteers.

S.H.I.N.E (Serving the Health Information Needs of  Elders)

  • Health insurance options from trained SHINE (Serving Health Information Needs of Elders) Counselors.
    • By Appointment on Tuesday between 9:00 and 12:00
    • Referrals to other Senior Centers providing the same service.

 

 FILE-OF-LIFE

The Outreach Department has developed a handy form for Seniors to place on the outside of their refrigerators that contains all pertinent information should a medical emergency arise. This free, magnetic holder and form is well-known to the emergency response teams in Hopkinton.  

The File-of-Life sheet contains information regarding contacting your closest relative or friend, your insurance account numbers and a list of medications you take, along with other information a hospital might need.  Please contact Joyce or Marlene in the Outreach Department if you would like to have this form.  And, if you already have one, please be sure to keep it up-to-date or ask for a new sheet.  If you need assistance in filling it out Outreach would be happy to help.


PALLIATIVE CARE VERSUS HOSPICE CARE

 Palliative care is specialized medical care for people with serious illnesses. It focuses on providing patients with relief from the symptoms and stress of a serious illness. The goal is to improve quality of life for both the patient and the family. Palliative care is provided by a specially trained team of doctors, nurses and other specialists who work together with a patient’s doctor to provide an extra layer of support. It is appropriate at any age and at any stage of an illness whether it be terminal or not and can be provided along with curative treatment.

Hospice care is compassionate care for people facing a life limiting illness (terminal or within 6 months of death).  It provides a team orientated approach to expert medical care, pain management; emotional support and spiritual support tailored to the patient's needs and wishes. Hospice care concentrates on comfort rather than aggressive medical treatment. By electing to forego life prolonging treatment, hospice patients can concentrate on getting the most out of the time they have left, without some of the negative side effects life prolonging treatment can have.

Both palliative and hospice care require a physician's order and are covered by most insurance plans. These services can be administered in a patient's home environment as well as an institutional setting. If you or a loved one have been diagnosed with a serious illness and could benefit from palliative or hospice care, please speak with your doctor. At the center of palliative and hospice care is the belief that each of us has the right to die pain free with dignity, and our families will receive the necessary support to allow us to do so.
